What should I bring to my first appointment?



Please bring your photo ID, current insurance cards, up-to-date medications list and any recent medical records.



When should I arrive for my first appointment?



I suggest arriving just a few minutes before your scheduled time in case you have any questions, want to update any information, or to simply relax for a minute or two before we get started. There's never any rush.



Do you need my previous medical records?



If you have your records, great. If not, we will request them from your various providers to make certain we have the most up-to-date information about the state of your health.



How does my insurance work with your MDVIP Practice?



We still accept most major medical insurance including Medicare. Just click on the button below to see if your plan is listed. If not, feel free to call us for more information. Your Annual Fee pays for preventive care medical services that insurance usually doesn’t cover (i.e., advanced diagnostic testing and screenings). We will continue to bill your insurance and charge copays, co-insurances and deductibles for other medical services like sick visits.


Accepted Insurance

My current insurance already pays for an annual physical, so why do I need the Wellness Program?



Our Wellness Program includes lab tests, screenings and diagnostics that go beyond what is typically included in a basic physical exam. These tests, for example, have been shown to identify 40 percent more people at risk for heart attack or stroke compared to standard cholesterol tests. The detailed results of this additional testing, in combination with our one-on-one counseling, will be used to develop your customized wellness plan. Where do I get my labs done?



For your convenience, we have our own phlebotomist and lab work is processed through Quest.



Do you have same day appointments?



As an MDVIP-affiliated practice, we guarantee same-day or next day appointments. It's just one of the many benefits of your MDVIP membership.



How do I get refills for my medications?



In many (most) cases, your pharmacy will contact us on your behalf for routine refills. Or, you may simply call us anytime for refills.



What if I have to cancel my appointment?



We understand that sometimes things come up at the last minute or out of your control. Just give us a call and we'll gladly change your visit to suit your busy schedule.
