What should I bring to my first appointment?

Please bring your photo ID, current insurance cards, up-to-date medications list and any recent medical records.

When should I arrive for my first appointment?

We ask that you arrive 30 minutes prior to your scheduled first appointment time. For subsequent visits, please arrive 5 minutes prior to your appointment. This gives us time to complete any necessary paperwork or update information. We appreciate your efforts and cooperation.

Do you need my previous records?

Yes. We will send a records request to your previous provider. One of the forms you'll be asked to complete grants permission for the records request.

Do I have to pay at the time of service?

Yes. It's a part of your contract with your insurance carrier that you pay co-pays, co-insurance and deductibles at the time of service.

Where do I get my labs done?

Most patients use LabCorp or Quest, but you can also have labs drawn at any of the local hospitals. It's a good idea to check with your insurance carrier to determine which facilities they cover. Please have lab done one week prior to your appointment so we'll have the information available during your visit.

Do you have same day appointments?

We try our best to accommodate our patients and get them in as soon as possible, especially if they are sick. If you need a same day appointment, please call us first thing in the morning. We'll do our very best.

How do I get refills for my medications?

You can call and leave a message on our refill line. Please leave all pertinent information including your name, the medication and dose, and the pharmacy. Also, your pharmacy can send us a refill request. Refills are typically completed within 2 business days.

What if I have to cancel my appointment?

Please call us at least 24 hours before your appointment if you need to cancel. Of course we understand that sometimes things come up at the last minute, but 24 hour notice is greatly appreciated..
